EVERYONE is eligible and approved.
Members must remain in the plan for a minimum of 12 months.
Membership fees are non-refundable once any treatment is performed.
Surgical, Non-surgical and/or Laser periodontal therapy (deep cleanings) are eligible for the 25% discount, but are not included in the two (2) routine cleanings the plan provides.
